Frequently Asked Questions

Please reach out if you cannot find an answer to your question.

The Conversation Nurse helps individuals and families prepare for healthcare decisions before a crisis happens. Through education, planning, and advocacy, we bridge the gap between the medical system and the human experience—so wishes are understood, respected, and honored.

Our services are for adults of all ages, especially:

  • People planning ahead for aging
  • Those living with chronic or serious illness
  • Families caring for aging loved ones
  • Anyone who wants clarity and peace of mind before a medical crisis
We also work with hospitals and organizations seeking better support for patients and families.

Serious illness refers to conditions that affect daily life, independence, or long-term health—such as progressive or life-limiting illnesses, advanced chronic disease, frequent hospitalizations, or increasing reliance on others for care.

Advance directives are important—but they are only a starting point. Real-life medical decisions are nuanced and often made in stressful situations. We help people go beyond documents by clarifying goals, preparing families, and translating wishes into real-world decisions.

We guide participants step by step through understanding medical choices, clarifying values, selecting a healthcare surrogate, completing documents, and preparing for hospital care—so decisions are informed, confident, and aligned with what matters most.

No. In fact, planning before illness or crisis is when it’s most effective. Many people work with us while they are healthy, aging, or caring for a loved one.

Legal and financial planning focus on paperwork. We focus on real-world healthcare decisions, communication, and advocacy—ensuring documents reflect medical reality and are understood by families and clinicians.

It’s a practical, bedside-ready resource that helps patients and families navigate hospital care with clarity—understanding treatments, asking the right questions, and advocating for care that aligns with their goals.

Yes. We partner with hospitals, clinics, and community organizations to provide education, workshops, and licensed resources that improve patient experience, reduce crisis-driven care, and support goal-concordant decision-making.

You can schedule an individual planning session, join/request a workshop, purchase the Serious Illness Hospital Guide, or simply reach out with questions. There’s no pressure—just guidance and support.

We complement existing legal, financial, and clinical services by helping clients understand real-world healthcare decisions, clarify goals and values, and prepare for medical crises. This reduces confusion, improves alignment, and enhances the overall quality of support you provide—without duplicating your work.

We partner with community groups, faith organizations, senior centers, and workplaces to offer educational seminars and workshops tailored to your audience. We handle content and facilitation; you provide the space and outreach. Workshops can be customized for size, timing, and focus.

Hospitals can license the Serious Illness Hospital Guide for patient and family distribution. Licensing options allow organizations to offer the guide digitally or in print to support communication, reduce crisis-driven care, and improve patient experience and goal-concordant outcomes.
